Première Urgence - Aide Médicale Internationale (PU-AMI) is an international, non-profit, non-political, and non-religious humanitarian organization which aims to provide rapid global response to the basic needs of populations affected by humanitarian crises to help them regain independence and dignity. PU-AMI provides assistance to more than 6 million people throughout 24 countries, with more than 2,500 staff worldwide.
PU-AMI adopts an integrated approach, to identify and understand the full array of the needs of the people affected by crises. This innovative approach is at the core of every PU-AMI-supported projects, to better target and prioritize actions, stabilize and improve the situation of the most vulnerable groups. It allows the teams to take all aspects of a problem into account, to come up with an efficient combination of solutions, which will have a powerful and long-lasting impact on the affected populations. In this respect, the organization runs around 190 projects a year worldwide, most of which are focused on Health, Nutrition, Food Security, Water, Sanitation and Hygiene, Psychosocial Support, Education and Protection activities. 
In Yemen, PU-AMI is supporting 18 health facilities (Health Centers, Health Units and Therapeutic Feeding Centers) in four Districts in Hodeidah (Al Mighlaf, As Sukhnah, Al Mansuriyah and Al Qanawis), three Districts in Raymah (Bilad At Ta’am, Al Jabin and Mazhar) and two Districts in Taizz (Al Mokha and Mawza). PU-AMI’s operations are managed by two field offices in Hodeidah and Mokha cities, both supported by the Sana’a coordination office. PU-AMI recently established a second coordination office in Aden. 
PU-AMI is participating actively in clusters and sub-clusters (health; nutrition; food security and livelihoods; WaSH) and works closely with relevant government departments.

Job Description

Overall objective

The TFC medical doctor (TFC MD) Officer will help the TFC team leader to organize, supervise, and support the entire clinical team, including GHO Medical doctors;  GHO auxiliary nurses, health educator other non-medical staff members in the smooth running of the TFC. The TFC MD ensures that daily clinic activities follow PU-AMI’s technical standards, protocols, policies, and procedures. 
In addition to the supervision duties, the TFC MD is also be expected to perform clinical duties relevant to his/her role as a medical doctor/physician ensuring the sound management of the Patients admitted at the Therapeutic Feeding Center with Severe Acute Malnutrition and Complications as per CMAM’s TFC Protocols.  S/he works in collaboration with the Pediatric Unit at MOPHP Health center/Hospital to integrate other Components of CMAM.

Tasks and Responsibilities

1.    S/he manages the implementation of the nutrition and health technical services in the TFC as defined by CMAM’s TFC Protocols 

2.    S/he achieves a high standard of care for patients in the TFC in collaboration with all members of the multidisciplinary clinical team.

3.    S/he effectively coordinates PU-AMI TFC activities with the Pediatric Unit at MOPHP Health center/Hospital. 

4.    S/he establishes and maintains linkages between TFC and other Components of CMAM.

5.    S/he participates in the capacity building of the government Health Workers 

6.    S/he helps TFC team leader to supervise and follow all other TFC staff during duty. 

7.    S/he helps TFC team leader to supervise all TFC activities and helps TFC team leader to prepare all TFC reports and needs on Time. 

Specific Goals and Related Activities

1.    MANAGE THE IMPLEMENTATION OF THE NUTRITION AND HEALTH TECHNICAL SERVICES IN THE TFC AS DEFINED BY CMAM’S TFC PROTOCOLS 

  • S/he ensures that all necessary equipment, drugs, and consumables are available, and in good working condition; and anticipates any needs ahead of shortages. S/he must inform the TFC team leader immediately of any shortages of drugs, consumables, and medical equipment. 
  • S/he is responsible for the implementation of the TFC part of the nutrition components at the MOPHP Hospital/Health center, including referrals from PU-AMI supported OTP sites. 
  • S/he is responsible for the management of SAM children with complications and provide technical guidance and training to the TFC team (medical and non-medical staff) as well as ensure protocols are strictly adhered to and the TFC is operating within quality indicators.  
  • S/he undertake emergency procedures (including resuscitation) for children on admission as may be required.
  • S/he validates the proper record of the malnourished patients by nurses as per MoPHP tools (registers, cards, databases, etc.). 

2.    ACHIEVE A HIGH STANDARD OF CARE FOR PATIENTS IN THE TFC IN COLLABORATION WITH ALL MEMBERS OF THE MULTIDISCIPLINARY 

  • S/he ensures all staff adhere to infection prevention and control (IPC) and medical waste management policies and protocols, medication administration and storage procedures, and controlled substance regulations, as per PUAMI SOPs, national / MoPHP and international guidelines. 
  • S/he works closely with PU-AMI Nutrition team (TFC team leader, Nutrition Supervisor and C4D team, PUI MD officers), MoPHP staff in transfer of child to OTP at Health Facilities. 
  • S/he works closely with the TFC team to see availability and quality of care-takers food / meals during their stay in the TFC. 
  • S/he works closely with the PUAMII MBC Nurse to make sure Mother and Baby Corner (MBC) services and activities are conducted well to mothers / caregivers and babies in the TFC.
  • S/he ensures that all health, nutrition, and hygiene promotion education sessions provided by the TFC Health Educator and other clinical staff members are conducted to a high standard utilizing national (MoPHP) health education, hygiene promotion and IYCF protocols, approved by TFC team leader. 

3.    EFFECTIVELY COORDINATE PU-AMI TFC ACTIVITIES WITH THE TFC DEPARTMENT AT MOPHP HEALTH CENTER/HOSPITAL

  • S/he will help TFC team leader with  close communication and liaison between the MoPHP hospital pediatric department and TFC for management of complicated cases of Severely Acute Malnourished requiring specialist follow up, reporting to the TFC team leader immediately of any issues. 
  • S/he coordinates with the MoPHP Hospital / TFC Department for referral of potential cases of severe acute malnutrition with complications to higher secondary health care facility / specialized care. 
  • S/he is responsible for coordinating with PU-AMI Medical Doctor Officers at the supported HFs for the referrals from PU-AMI supported OTP sites, as per PUAMIs Referral and Nutrition SOPs.

In collaboration with PU-AMI TFC Nurse ensure the proper discharge of the admitted cases and referral them to the concerned OTPs.

4.    ESTABLISH AND MAINTAIN LINKAGES BETWEEN TFC AND OTHER COMPONENTS OF CMAM 

  • S/he ensures that the health facility team, CHVs and community outreach workers work together and follow the effective referral systems established between OTP and TFC / stabilization center, reporting any issues to the line manager. 
  • S/he follows PU-AMI Referral SOPs when transferring patients.
  • S/he works closely with TFC team leader to ensure the TFC is running well and coordination among PU-AMI integrated projects related to the TFC are maintained. S/he supports and work closely with the MEAL team during their M&E visits to the TFC and facilitates the reception of the CRM mechanism to the MEAL team. 

5.    PARTICIPATE IN THE CAPACITY BUILDING OF THE GOVERNMENT HEALTH WORKERS 

  • S/he plans and implement training courses on different health and nutrition topics on weekly/biweekly basis for the PU-AMI and GHO TFC medical and non-medical staff according to MoPHP National Guidelines and PU-AMI SOPs.
  • S/he identifies and conducts any relevant on-the-job training required for the TFC staff. 
  • S/he prepares TFC staff for any external training that may be needed to enhance their capacity.
  • S/he assesses and provides the TFC team leader monthly, the training gaps and needs of the staff. 

6.    HELP IN SUPERVISION AND FOLLOWING UP ALL OTHER TFC STAFF DURING DUTY

  • S/he is ultimately responsible for the follow-up of all activities and tasks required from the staff (medical and non-medical) and supporting them to participate in their capacity building and communicating with the TFC team leader of any issues. 
  • S/he ensures that all staff have access to the relevant international, national, and PU-AMI specific guidelines/protocols to conduct themselves to a high standard in the line of duty (i.e., National CMAM guidelines, PU-AMI Nutrition, Referral, and IPC SOPs, WHO growth charts, etc.). Immediately notify the TFC team leader of any needs. 

S/he ensures that all staff have the necessary equipment to carry out their duties to a high standard and are all in a good working condition (i.e., MUAC tapes, glucometer, sphygmomanometer, etc.). Immediately notify the TFC team leader of any needs.
7.    HELP IN SUPERVISION ALL TFC ACTIVITIES AND SEND ALL TFC REPORTS AND NEEDS TO TFC TEAM LEADERON TIME

  • S/he ensures daily, weekly, and monthly reporting is completed and sent to the TFC team leader in a timely manner. 
  • S/he is responsible for the TFC Referral transportation allowance advance and monitors and clears the advance in accordance with PU-AMI procedures. 
  • S/he anticipates all TFC needs and immediately notifying the TFC team leader of any upcoming shortages or issues. 
  • In General, the TFC MD will help TFC team leader to ensure the motivation and cohesion of the TFC team, remaining flexible performing other duties as required for the good care of the patients. 
  • S/he has a weekly meeting plan with the TFC staff and to send the meeting minutes to TFC team leader. 
  • S/he ensures regular communication with the TFC team leader and immediately notifying the TFC team leader of any issues.
