Duration:   to 31st  of March 2018
Assignment: Overall responsible for SAM inpatient management activities through MoPHP hospitals including technical capacity building nutrition and health service delivery through the ministry of health facilities including technical capacity building and reporting.

MAIN RESPONSIBILITIES:

Objective 1: Ensure smooth implementation and proper functioning of Therapeutic Feeding Centre in Hospitals

Tasks and Responsibilities:
Detection, referral and assessment of acute malnutrition

  • Ensure that facility based screening, diagnosis, detection of acutely malnourished children/sick children is in place at the hospital
  • Ensure that the referred acutely malnourished/sick children from health facilities or community health workers/volunteers are re-diagnosed and documented as referred by health facilities or community health workers/volunteers
  • Ensure that adequate health checkup is done to identify medical complications according to IMCI protocol
  • Ensure that appropriate referral of children is maintained within the CMAM components (OTP, SFP, SC and IMCI) based on anthropometric and medical assessments

Ensure admissions and discharge of eligible beneficiaries (severe acute malnutrition cases with medical complications) in the SC.

  • Support MoH nurses to decide on admission inpatient/referral to OTPs based on medical and anthropometric assessment.
  • Decide which patients should be admitted rapidly with an abbreviation history and examination after formal admission and the start of treatment.
  • Ensure the dispensing of sugar water and F75 upon admission in SC
  • Provide technical support and coaching to MoH nurses for monitoring and diagnosis of hypothermia, hypoglycemia, dehydration, and severe anemia, take timely action to prevent and treat those complications using the protocol strictly and in consultation with the medical doctor in charge.
  • Follow up of the anthropometric measurement of the children (height, weight, MUAC, checking bilateral oedema and W/H percentage) taken by registrar/measurer.
  • Record all the necessary information on the registration book, treatment follow up cards every day including admission and discharge.
  • Ensure admission and discharge criteria implemented is correct and in accordance with standard protocol (National CMAM/ACF guideline if National guideline is not available). 

Follow up of treatment phases  in the Stabilization center

  • Provide adequate coaching and hands on training to MoH nurses on the treatment and follow up protocols, procedures in the SC
  • Ensure that there is available nurses 24 hours for follow up  of cases and provision of medical and nutrition treatment to the beneficiaries
  • Provide technical coaching to MoH nurses on calculation, preparation of therapeutic milk and distribution and monitoring of feeding according to the standard protocol for the SAM beneficiaries.
  • Ensure that SC nurses are encouraging breastfeeding practices
  • Train MoH nurses on supplementary Suckling technique (SST) and ensure SST for mothers facing breastfeeding challenges with infants.
  • Follow up of the anthropometric measurements taken by registrar according to the protocols, daily, each other day or on weekly bases and record in the registration book, follow up cards for each beneficiary.
  • Ensure that the systematic treatment and drugs are given correctly to the beneficiaries.
  • Identified patient who need to be transfer to another phase or OTP, patient with failure to respond to treatment and patients who need to be discharged.
  • Follow up of the gain weight and feedback to the caregivers 
  • Follow up of the hygiene and sanitation rules in the centre and during treatment follow up.
  • Identification of absence and defaulters from stabilization centre and follow up at home through volunteers network and/or NUT/MH outreach staff

Objective 2: Drugs and therapeutic feeding products stock management

  • Keep daily record/tally on utilization of each medicine and therapeutic feeding product used in SC
  • Prepare weekly/monthly utilization, requisition and stock report for therapeutic feeding products and medicines used in SC
  • Liaise with pharmacist and TFC Supervisor/Deputy PM Health for supplies with adequate anticipation
  • Follow up the utilization and expiry information of all therapeutic feeding product and medicines at regular basis and inform supervisor for appropriate action required
  • Support that the order forms are in conformity (including consumption coherence) and ensure anticipation to prevent shock out.
  • Follow up on food for care taker allowance and support CT meal preparation, occasion for health education: prepared respecting the hygiene rules. Ensure/encourage balance food intake.
  • Provide support in milk preparation in accordance with protocol and hygiene rules.
  • Manage admin and logistic needs to keep unit functional for whole month in absence of the field log support.
  •  Ensure IT, electronic and nutrition equipment are functional and standardized or repaired whenever required.

Objective 3: Ensure maintenance and hygiene for all equipment and premises.

  • Ensure adequate hygienic conditions of the facilities and provide adequate means (soap, water) for hand washing of SAM beneficiaries, caretakers during appetite test
  • Ensure that all equipment is used in an appropriate manner by those present at the centre;
  • Verify the availability of clean drinkable water for patients and activities. In case of any problem liaise with WaSH team through line manager.
  • Liaise with WaSH team for appropriate removal procedures for sewage, solid waste and medical material
  • Liaise with WaSH team and update a check-sheet for each point (quantity, quality, water chlorination).

Objective 4: Team Management

  • Participate in joint supervision visit with MoH supervisor/in-charge
  • Participate in Drawing up work schedules for each ACF/MoH nurses in close collaboration with hospital in charge;
  • Update ACF/MoPHP SC staffs attendance, check attendance and validate by hospital in charge;
  • Identify training needs on in patient treatment for ACF/MoH staff and report to supervisor;
  • Organize weekly and monthly meetings with ACF/MoPHP staffs in Stabilization Centre;
  • In the event of problems, identify the issue, seek solutions from supervisor and coordinate with hospital in charge or refer up to management superior;
  • Verify the manner in which the team interacts with beneficiaries and assistants and support improvements;
  • Ensure transmission time between shift teams.

Objective 5: Information management, reporting and coordination 

  • Maintain supervision checklist periodically for Inpatient facility
  • Ensure that data collection template for CMAM database is filled and shared with line manager/data entry officer every month.
  • Maintain and keep TFC quality monitoring checklist during assessment
  • Collect monthly report for SAM treatment and share it with line manager
  • Analyze complied reports and performances for the targeted health facilities and share summaries with line manager
  • Closely coordinate with WaSH team to ensure appropriate WaSH facilities in the hospital within the framework of the project
  • Regularly report and coordinate with line mangers regarding challenges, issues faced in the field for timely and appropriate decision making.

Objective 6: Working with emerging needs in other operational areas for emergency responses or with another mobile teams in case of any gaps

POSITION REQUIREMENTS

QUALIFICATIONS

  • Qualified Medical Professional/Medical doctor with at least 2 years MOPHP or private level experience in Pediatrics unit.

Skills requarid :

  • At least 1 year experience in the field nutrition and health project implementation (CMAM, SFP/OTP/SC, IMCI)  preferred or care & feeding practices), 
  • Knowledge and experience in pediatric ward
  • Good monitoring and supervision skill is essential
  • Knowledge on management of childhood illness protocols 
  • Ability to observe and to provide feedback on the activities
  • Ability to understand and to interpret the nature of human behavior
  • Ability to show activeness and creativity in working environment
  • Ability to use fluently Word, Excel, Power Point a plus
  • Ability to read, write and understand English is essential
  • Ability to work under pressure and react according to evolving context/situation

ESSENTIAL

  • At least 1 year experience in the field nutrition and health project implementation (CMAM, SFP/OTP/SC, IMCI)  preferred or care & feeding practices), 
  • Good monitoring and supervision skill is essential
  • Knowledge on management of childhood illness protocols
  • Ability to read, write and understand English is essential

PREFERRED

  • Experience in implementing IMCI programs including CMAM is a plus
  • Ability to observe and to provide feedback on the activities
  • Ability to show activeness and creativity in working environment
  • Ability to work under pressure and react according to evolving context/situation
  • Team management and organization skills are preferred
  • Ability to use fluently Word, Excel, Power Point is a plus
  • Ability to understand and to interpret the nature of human behavior
  • Pediatric Advanced Life Support (PALS) certification is preferred or to be obtained within six (6) months of recruitment.
